Provides and documents case management for clients in Urban Pathways' Scattered Site supportive housing program for formerly homeless adults, connecting them to entitlements, housing, employment, and health or substance-abuse services. Conducts crisis intervention and daily-living-skills support, and develops individualized service plans toward client self-sufficiency. Hybrid schedule: 4 days in office, 1 day remote.
