Service Coordinator

Service Coordinator working with senior tenants at Project FIND's Woodstock Hotel supportive housing residence. Conducts intake assessments, connects residents to benefits and entitlements, and provides ongoing case management, counseling, and crisis intervention. Works with the property management team on income affidavits, rental subsidy maintenance, and recertifications, and coordinates with healthcare providers and family caregivers.
