Evening case management position assisting veterans in Housing Visions' permanent supportive housing, part of the CONNECT program. The coordinator assesses needs, develops service plans, and provides case management, advocacy, and referrals to help veterans secure and maintain stable housing. Full-time, $25-$28/hour, with travel throughout an assigned portfolio.
